Student Transportation Request Form for Parents/Guardians with Shared Custody

Parents/Guardians may request alternate pick-up and/or drop-off locations for a student whose custody is shared by two separate parties. Parents/Guardians must agree on a total of two designated bus stops and may not request two bus stops each. For safety reasons, it is expected that the pick-ups and drop-offs at each parent's/guardian's location will remain consistent as it is not possible to flex days and locations on an ongoing basis.

Please fill out the form below and scan and/or email the form to Tina Murray at tmurray@bonnyeagle.org, mail the form or bring it to the MSAD 6 Transportation Department located at 715 Bonny Eagle Road, Standish, ME 04084. After verification by the School Administrator and review by the Transportation Manager, transportation will be assigned. All requested bus stop locations for elementary students must be located within the attendance zone of the school that the student is currently enrolled.
